口蓋扁桃におけるクラミジア陽性率の検討

Incidence of Chlamydia Trachomatis Infection in Palatine Tonsil

　はじめに

　クラミジアは，DNA，RNAの2種類の核酸を有し，宿主細胞に取り込まれ細胞内で増殖をくり返す微生物である。泌尿器科や産婦人科領域の非淋菌性尿道炎や子宮頸管炎などは，これが原因で発症すると報告されている^1）。頭頸部領域におけるクラミジアは，扁桃炎や咽頭炎患者さらに中耳炎患者からの検出が報告されている^2〜4）。

　扁桃炎や咽頭炎患者からのChlamidia trachomatisやChlamidia peumoniaeの検出率には，報告により差が認められる。すなわち，Jonesら^2）は3.5％，McDonaldら^3）は6.3％と報告しているが，小川ら^5）は，扁桃陰窩から検体を採取した場合のC．tracho-matisの検出率は細胞培養法で26.7％であるとしている。さらに，これらの検出率の相違は，検体採取方法の違いによるものであるとしている^5）。

C. trachomatis was cultured in 181 patients with tonsillitis. Swabs were taken from the superior tonsillar fossa or plaque of tonsil for enzyme immunoassay (EIA). Of the 181 patients, 42% was positive after screening test by EIA (Chlamy-diazyme), and 3.3% after confirmatory blocking assay of Chlamydiazyme. The confirmatory test increased the positive result, and the test was thought to be useful for detection of C. tracho-matis.
