Werner症候群に合併した甲状腺癌—老化と癌化について

A Case of Werner's Syndrome with Thyroid Carcinoma

　はじめに

　老化と癌化は古くて新しい問題である。Werner症候群は若年期より白髪，白内障の老人様症状がみられる遺伝的背景を持つ疾患である^1）。思春期の急激な身長増加を欠いた均整のとれた発育障害を特徴とし，早期老化症候群の代表的な症候群としてヒトの老化モデルとして注目されてきた^2）。その約10％に悪性腫瘍を合併することが知られ，癌好発遺伝疾患の1つに数えられている^3）。われわれは甲状腺癌を合併したWerner症候群の1症例を経験した。その病理診断は腺内の多発性濾胞腺腫で，その内の2個の腫瘍は腺腫内に乳頭癌がみられる“carcinoma in adenoma”であった。老化と癌化，癌の組織発生を考える上で興味がある症例と考え，検討を加えたので報告する。

A 36-year-old male was diagnosed as having Werner's syndrome, which is one of the progeric syndrome, by clinical findings and symptoms. He had tumors in the anterior lower neck since he was 18 years old. Those tumors were removed by total thyroidectomy in our hospital. Histological examination revealed multiple follicular adenomas in the bilateral thyroid lobes. In two of adeno-mas, papillary adenocarcinomas enclosed with adenomas were observed. We discussed the rela-tionship between aging and canceration, and the histogenesis of papillary adenocarcinoma in this case.
