Orbital emphysema is generally benign and requires no treatment. However, if a fracture produces a ball-valve effect allowing air to enter but not to leave the orbit, extremely high in-traorbital pressure and visual loss is possible. A 23-year-old man was hit his face by a ball. After nose blowing, he developed right facial swelling and high intraorbital pressure. Com-puted tomography demonstrated orbital emphy-sema and fracture of floor of the left orbit. In order to avoid visual loss, transnasal orbital decompression was performed, and the intraorbital pressure was successfully decreased. A post-operative computed tomography showed improve-ment of orbital emphysema.
