Abstract Purpose:To report a case who developed transconjunctival emphysema caused by compressed air while using a spray gun.

Case:A 60-year-old male was accidentally injured in the face while working with a high-pressure spray gun. When seen half an hour after the accident, his visual acuity was 1.2 in either eye. His fece, including the right eyelids and cheek, showed swelling. The right eye showed a rupture in the conjunctiva temporal to the cornea. Numerous air bubbles were present in the subconjunctival space. Computed tomography(CT)showed orbital emphysema and blowout fracture in the nasal orbital wall. The orbital emphysema disappeared spontaneously without residue 3 days later.

Conclusion:This case illustrates that orbital emphysema may develop when working with a spray gun using compressed air.
