中咽頭まで下垂し嘔吐反射を反復していた鼻腔内反性乳頭腫の1例

A case of a nasal inverted papilloma that descended to the oropharynx and resulted in repeated vomiting reflex

はじめに

　鼻副鼻腔内反性乳頭腫は病理組織学的には良性腫瘍であるが，局所再発率が高く，悪性腫瘍の合併がみられる場合もあり，腫瘍基部を含めた完全切除が必要である。また，中咽頭まで下垂した鼻腔腫瘤は，鼻症状に加え，咽喉頭症状もきたしうる。今回，嘔吐反射を主訴に受診し，右鼻腔から中咽頭まで下垂していた内反性乳頭腫を経験した。腫瘍基部を含めて切除したのちに鼻腔から摘出し，症状も消失したので，考察を交えて報告する。

　Nasal sinus inverted papillomas are generally histopathologically benign but have a high local recurrence rate and may be associated with malignancy. Therefore, complete resection from the base of the tumor is considered necessary. Additionally, if the nasal mass descends to the oropharynx, it may cause nasal as well as pharyngolaryngeal symptoms. In this case, an inverted papilloma descended from the right nasal cavity to the oropharynx of a patient who presented with the chief complaint of repeated vomiting reflex. After resection and removal of the tumor from its base, the patient's symptoms resolved.
