cavity problemを伴ったopen法術後耳に発生した扁平上皮癌の2例

Squamous cell carcinoma arising in ears with cavity problems after open mastoidectomy

はじめに

　open法術後耳におけるcavity problemはしばしばみられる病態である。間歇的に耳漏が再発するものの，保存的治療により寛解するため，手術適応とは判断されず長期間にわたって通院する症例は稀でない。今回われわれは，上記のような経過をたどっていたものの耳漏の増悪から手術適応判定のために紹介を受け，結果的に扁平上皮癌が確認された2例を経験した。見落とされる可能性も高い病態であり，臨床的に重要であると考え，これらを報告し診断についての考察を加える。

　Two cases of squamous cell carcinoma arising in ears with cavity problems post-operatively were reported. Case 1：An 88-year-old woman had undergone open mastoidectomy at the age of seven. She had intermittent otorrhea for about 20 years, which became resistant to topical treatment for one year. Biopsy of the granulation was repeated four times, which finally revealed squamous cell carcinoma. Case 2：An 80-year-old woman had undergone open mastoidectomy at the age of 10. She suffered from intermittent otorrhea for about 15 years and developed facial palsy one year ago. Biopsy of the granulation revealed squamous cell carcinoma. The difficulty in diagnosis was also discussed.
