上咽頭腫瘤を呈した斜台脊索腫

A case of nasopharyngeal clivus chordoma

はじめに

　脊索腫は，遺残した胎生期の脊索から発生すると考えられている稀な悪性骨軟部腫瘍である。発生部位はその機序から，仙骨，斜台骨，脊椎などの軸骨格が中心となるが，頭蓋底に発生した脊索腫は上咽頭腫瘤として遭遇する可能性がある。今回われわれは，偶発的に見つかった上咽頭腫瘤により，脊索腫の画像診断に至った症例を経験したため，文献的考察を加えて報告する。

　Chordomas are relatively rare malignant neoplasms derived from the embryonic remnants of the notochord. This explains why chordomas usually occur in the axial skeleton such as the skull base, vertebra, and sacrum. When occurring in the skull base, chordoma may be observed as a nasopharyngeal tumor. We report the case of an 80-year-old female with an asymptomatic nasopharyngeal mass, which was finally diagnosed as chordoma based on the characteristic findings on computed tomography and magnetic resonance imaging.
