広範な梗塞を伴った多形腺腫の1例

A case of parotid gland pleomorphic adenoma with extensive infarction

多形腺腫は唾液腺腫瘍で最も頻度が高く，通常緩徐に増大する充実性腫瘍であり，多彩な組織像を呈するが，広範な梗塞を伴うことはまれである。今回我々は，右耳前部腫脹を主訴に発見され，経過中に増大と縮小を示し，病理組織像で広範な梗塞後変化を認めた多形腺腫の1例を経験したので報告する。

We report a rare case of parotid gland pleomorphic adenoma with broad necrosis caused by extensive infarction. A 42-year-old man complained of his right parotid gland swelling with tenderness. The palpation and the ultrasound examination demonstrated a right parotid gland mass. Contrast-enhanced MRI showed rim-enhancing right parotid mass. A mass shrank slightly, but to make a pathological diagnosis, a tumorectomy was performed. A pathological examination revealed broad central necrosis, and a small amount of tumor tissue beneath the fibrous capsule. A histological diagnosis of infarcted pleomorphic adenoma was made.
