肺MAC症の経過観察中に生じた鼻腔・顔面皮膚・肺結核例

A case of tuberculosis in the nasal cavity, face, skin, and lungs occurred during a follow-up for pulmonary MAC disease

はじめに

　本邦の結核患者は減少傾向で，結核罹患率（人口10万対）が10.0以下となり，2021年に結核低蔓延国になった。今後もこの減少傾向が続くと予想される^1）。

　肺外結核のなかでは，頸部リンパ節結核は散見されるが^2），鼻副鼻腔領域の結核は少なく，遭遇する機会は少ないため，疑わなければ診断は難しい。しかし，診断が遅れると患者への不利益のみならず，同居者や医療従事者へ伝播することも否定できない疾患であることを理解する必要があることに変わりはない^3）。今回われわれは，肺非結核性抗酸菌症〔以下，肺Mycobacterium avium-intracellulare complex（MAC）症〕の経過観察中に生じた鼻腔・顔面皮膚・肺結核例を経験したので，文献的考察を踏まえて報告する。

　Tuberculosis（TB）continues to be a public health problem in developing countries, but primary nasal and facial skin tuberculosis are rare presentations of extrapulmonary tuberculosis. Japan is expected to become a low-incidence country by 2021. A 90-year-old Japanese woman presented to the ENT service with a 5-month history of nasal obstruction and facial skin lesions. The diagnosis was based on a smear study, histopathology, culture, CT, and polymerase chain reaction. Combined chemotherapy with isoniazid, rifampicin, pyrazinamide, and ethambutol was administered, and the lesion disappeared.
