Grade 3，4突発性難聴における高気圧酸素療法の有用性の評価

Evaluation of hyperbaric oxygen therapy in sudden hearing loss of Grade 3 or 4

はじめに

　突発性難聴（idiopathic sudden sensorineural hearing loss：ISSHL）の治療として，ステロイド療法，高気圧酸素治療（hyperbaric oxygen therapy：HBOT），鼓室内ステロイド療法，ビタミンB₁₂内服などが施行されているが，いずれも十分なエビデンスがあるわけではない。当科ではISSHLに対しては，基本的にはステロイド療法とHBOTの併用療法を施行している。しかしHBOTは，呼吸器疾患の既往のある症例，閉所恐怖症，また頻回の通院困難などを理由に施行できない場合がある。今回われわれはISSHLの治療に際して，HBOTを施行しなかった症例（以下，ステロイド療法群）を施行した症例（以下，併用療法群）と比較し，HBOTのステロイド療法への上乗せ効果を検討し，文献的な考察をふまえて報告する。

　Objective：To evaluate the effectiveness of hyperbaric oxygen therapy（HBOT）in the treatment of patients with idiopathic sudden sensorineural hearing loss（ISSHL）.

　Material and methods：Patients with ISSHL were divided into two groups. Steroid therapy group consisted of 35 patients who were treated with steroids and without HBOT. Combination therapy group consisted of 67 patients who were treated with steroids and HBOT.

　Results：In Grade 3 of 8000 Hz ISSHL patients, significant benefit was observed in the combination therapy group. However, in Grade 4 of ISSHL patients, no significant difference was demonstrated between the two groups.

　Conclusions：The effect of HBOT is restrictive.
