Malleus bar内部に鼓索神経が走行する耳小骨奇形症例

A case of auditory ossicular malformation with chorda tympani nerve running inside Malleus bar

はじめに

　先天性耳小骨奇形はこれまで数多くの報告がなされ，その分類や手術療法に関して盛んに検討されてきた。とりわけ近年は，検診によるスクリーニングの普及により耳小骨奇形に遭遇する機会が増え，また内視鏡下耳科手術の発展に伴い奇形構造をより詳細に観察できるようになった。

　Malleus barは，骨性鼓膜輪とツチ骨柄を連結する骨梁の存在によって伝音難聴をきたす比較的稀な耳小骨奇形である^1）。手術加療においては，骨梁を除去することで聴力改善する症例が多い^2）が，顔面神経や鼓索神経の走行異常を合併している症例^2,3）では，手術操作に注意が必要である。

　今回われわれは，鼓索神経の走行異常により骨梁内部を鼓索神経が走行する症例を経験し，術後聴力改善を認めたので報告する。

　Malleus bar is a rare malformation of the ossicles characterized by a bony connection extending from the manubrium of the malleus at the posterior lip to the posterior wall of the tympanic cavity. In many cases, the removal of the bony connection improves conductive hearing loss. Herein, we report a case of a Malleus bar with tympanic nerve invasion in a patient with hearing loss that showed improvement after surgery.
