　はじめに

　熱傷や創傷時の被覆保護剤として，ポリ-N-アセチルグルコサミン（一般名キチン）繊維で作られた医用材料が広く使用されている。頭頸部領域においても，穿孔鼓膜に対する保存的穿孔閉鎖や中耳手術^1），副鼻腔手術後の鼻内タンポンとしての使用などが報告されている^1,2）。キチンは創面への密着性に優れ，生体親和性が高く，表皮形成を促進する作用などを有している^3,4）。そこでキチンのこれらの性質を利用し，鼓膜の穿孔部位にキチンを密着させ慢性中耳炎の鼓膜穿孔が閉鎖される割合について検討を行った。

A 0.08 mm thick cushion of Chitin (N-acetyl-D-glucosamine) was used for closure of a perfo-ration of the tympanic membrane which pre-sented for 6 months in 40 patients. After de-bridement of the perforation edge, the cushion was placed over the perforation for 6 weeks. Complete closure of the perforation was achieved in 42 %, and incomplete (perforation diameter less than 0.5 mm) in 23%. In 14%, the dia-meter became smaller but exceeded 0.6 mm and in 15%, no change of size of perforation was observed. In 20% otorrhea was recurred. This method is easy in the outpatient clinic.
