学校検診で発見された脊髄由来神経鞘腫の1例

A case of neurinoma spreading inside and outside the vertebral canal which was found in the school medical examination

はじめに

　頭頸部領域において神経鞘腫は珍しくない疾患である。頭頸部領域では迷走神経や交感神経幹，頸神経叢，腕神経叢などから発生することが多いが，症状が出ることはあまり多くはない。特徴的な症状として，腫瘍圧迫により迷走神経由来のものでは咳嗽が，腕神経叢由来のものでは上肢への放散痛が認められることがある。症状が少ないために発生母地神経を同定するには術中の神経同定や術後の脱落症状からしかなく，結局，発生母地神経が不明のままであることも多い。

　一方で頭蓋内や脊椎などの骨で囲まれる部位に発生すると神経症状が出やすい。そのため，より積極的に治療を検討する必要がある。われわれは，学校検診にて咽頭後壁の腫脹として指摘された脊柱管内外に拡がる神経鞘腫の1例を経験したので報告する。

　Neurinoma is not rare in head and neck region, and seldom cause symptoms in especially neck region. On the other hand in the region surrounded with bones, neurinoma causes symptoms easily.

　A 16-year-old male had suffered from mild motor weakness of the left hand and in medical examination it was found that his pharyngeal wall was swollen. The swelling was caused by a neurinoma spreading inside and outside the vertebral canal. The neurinoma was removed and after operation his symptom was improved.
