外耳道に発生した黄色肉芽腫の1例

Xanthogranuloma of the external auditory canal in an adult

Ⅰ．はじめに

　黄色肉芽腫（xanthogranuloma）は，主として乳幼児の頭部や顔面に好発するという特徴から，一般的に若年性黄色肉芽腫としてよく知られている^1）。しかし，同様の組織学的所見を示す成人発生例が1963年に報告されて以来^2），成人発症の黄色肉芽腫症例が多数報告されている^3）。黄色肉芽腫は頭部や顔面に好発するとされるものの，これまでの報告上，外耳に発生することは稀で，わが国では皮膚科から3例の報告があるものの^2,4,5），外耳道発生例の報告はない。今回，われわれは成人男性の外耳道に発生した黄色肉芽腫症例を経験したので文献的考察を加えて報告する。

　Xanthogranuloma,a form of non-Langerhans cell histiocytosis,of the external auditory canal is an extremely rare condition. We treated a 57-year-old Japanese man with xanthogranuloma in the left external auditory canal. The yellowish-brown and well-defined mass was completely excised under local anesthesia. Postoperative pathologic findings for the mass including the proliferation of foamy histiocytic cells admixed with multinucleated Touton giant cells and immunohistochemical positive reaction for CD68 and negative reaction for S-100 protein confirmed the diagnosis of xanthogranuloma. To our knowledge,this is the first reported case of xanthogranuloma of the external auditory canal in the Japanese literature.
