A 54-year-old female complained of left pro-gressive hearing loss and dizziness for six years.

Her physical examinations revealed a swelling of the left tympanic membrane and incomplete palsy of the XI and XII cranial nerves.

A pure tone audiogram showed a mixed hearing loss of 65 dB in the left ear. Plain CT scan showed a high density area in the temporal bone. Exploratory tympanotomy was performed and histopathological examination revealed meningio-ma.

After the operation, enhance CT scan, angio-graphy and MRI were performed and a large mass in the left temporal bone and the posterior fossa was found.

We considered that an intracranial meningioma involved the middle ear, invaded through the jugular foramen.
