　I．はじめに

　中耳真珠腫は先天性真珠腫と後天性真珠腫（1次性および2次性）とに分類されるが，鼓室に真珠腫があつてしかも鼓膜に穿孔が認められない場合には，先天性か1次性かにわかに断定を下しえないことがある。

　最近私どもは中耳炎の既往をもたず，鼓膜穿孔もないにもかかわらず一側の難聴とX線写真上で上鼓室から乳突洞に骨欠損を認めたために手術を行ない，大きな真珠腫を充満する症例を経験したので，いささかの考察を加えて報告する。

　A man, aged 17, complained of loss of hearing in the left ear. His history showed no previous attack of otitis media ; examination showed normal left ear drum. However, with X-ray examination there was a shadow in the upper portion of the tympanic cavity and abnormality in the inner ear ossicles. Equilibrium test showed CP in the left ear.

　Operation revealed cholesteatoma occupying the left ear.
