A case of Ludwig's angina caused by periapical cyst infection was reported. A 24-year-old woman suffered from fever, dysphagia, trismus, and the submandibular swelling. On examination, indura-tion of suprahyoid and floor of mouth, elevation and protrusion of the tongue, edema of left epiglot-tis, and fluctuation of the right submandibular glands were found. Orthopantomogram showed a periapical cyst of the right lower second mollar. Surgical drainage was done, and high doses of antibiotics and gamma globulin were administered. Submandibular swelling was ceased on the 4th day but laryngeal edema became worse, but sub-sided on the 5th day. Extraction was done on the 14th day.

Despite a reduction in preantibiotic mortality rates, Ludwig's angina remains a potentially lethal entity because of rapidly progressive airway obstruction. A bibliographic review was made and the necessity of monitoring of respiratory status was emphasized.
