syncopeを伴うvasospastic anginaの検討

Study on the vasospastic angina with syncope

　Prinzmetalらにより記載された異型狭心症^1）の特徴の1つは，発作中に種々の不整脈が出現することである。この不整脈は房室ブロックや危険な心室性不整脈であることが多く，Adams-Stokes発作や突然死をも来すことが報告されている^2〜4）。異型狭心症のみならず安静時狭心症の大部分が冠動脈攣縮により発作が生じ，本質的には同一のものであることが種々の冠動脈造影検査により確証された^5〜7）。Maseriら^8）はかかる狭心症を一括して冠動脈攣縮性狭心症（vasospastic angina）と呼んでいる。本論文では，狭心症発作中にsyncopeを呈したvasospastic angina症例につき，臨床所見，心電図および冠動脈造影所見の特徴を明らかにすることを目的とした。

In 48 patients diagnosed vasospastic angina, the clinical symptomes, electrocardiographic findings and coronary arteriographic characteristics includ-ing ergonovine provocative test were compared 12 cases with syncopal episodes and 36 cases without it.

The daily incidence and duration of anginal attack were significant higher and longer in patients with syncope than those without it.
