鼻粘膜広汎切除術：遠隔成績について

Extensive Turbinotomy for Allergic Nasal Obstruction: Long-Term Efficacy

I．はじめに

　鼻アレルギー症状の一つである鼻閉は鼻粘膜のmetachromatic cellから遊離したchemical media—torにより局所血流の循環障害や浮腫が生じた結果起こるものと，長期にわたって繰り返すアレルギー反応により結合組織の増生や血管の拡張が生じることによるものとがある^1,2）。前者は可逆性で薬物などの保存的治療により奏効する場合が多いが，後者は治療に抵抗する^1,3）。このような難治性の鼻粘膜の腫脹に対し従来より下鼻甲介粘膜広汎切除術を行ってきたが，手術後1〜2年は鼻閉は完全に消失するものの，長い経過をみると再び症状の悪化をみるものも経験する。著者らは下鼻甲介粘膜切除術後5年以上経過した症例をアンケートによって遠隔成績を求めた結果，術後2年まではよい結果が得られてもその後再発する例があり，とくに術前のくしゃみ，鼻汁の程度が高度なもの，またはこれらのコントロールできないまま手術した症例は鼻閉の再発をぎたしやすいという知見を得たので報告する。

Eighty seven patients with allergic rhinitis were operated on for nasal blockage by the extensive removal of the nasal turbinate mucosa. In 2 yearsafter the operation, the percentage of relief of nasal obstruction was over 90%, but it decreased to 70% in the patients who were operated on more than 5 years ago. Of 69 patients who were operated over than 5 years ago, 48 patients re-sponded to questionnaire about nasal symptoms. Thirteen patients had recurrence of the nasal blockage. These patients had more severe sneezing or nasal discharge at the time of the operation than the group without recurrence.
