下咽頭梨状窩瘻の治療経験

Piriform Sinus Fistula and its Treatment:Report of 4 Cases

　はじめに

　下咽頭梨状窩瘻（piriform sinus fistula）は，若年より反復する前頸部感染歴を特徴とする稀な疾患である。治療には瘻管の完全摘出が必要とされており，不完全な場合には術後も頸部感染症を反復することになり治療に難渋する。今回われわれは最近経験した梨状窩瘻4例を報告するとともに，主に治療に関し若干の考察を加える。

A piriform sinus fistula was resected with the thyroid gland in 4 patients. Indigo carmine was injected into the fistula under laryngomicroscope to find the duct easily and clearly in the surgical field. The distal end of fistula is not easily identified by any means, because it often reaches deeply into the thyroid gland. For this reason, we performed fis-tulectomy and lobectomy of the thyroid gland in all cases for complete resection of fistula. No recur-rence was observed after surgery.
