明らかな外傷の既往なく発症した破傷風の1例

A case report of tetanus without any apparent history of trauma

Ⅰ．はじめに

　破傷風は，外傷により感染し，破傷風菌（Clostridium tetani）が産生する神経毒素により開口障害，呼吸困難，後弓反張などを引き起こすが，治療が遅れると致命率が高い病態である。予防接種の普及により，発生率および死亡率が低下しているが，現在でも年間100人程度の発症がある^1）。開口障害を主訴に受診する患者は耳鼻咽喉科を受診する可能性があり，注意を要する。

　今回われわれは明らかな外傷の既往なく発症した破傷風症例を経験したので文献的考察を加えて報告する。

　A 68-year-old-woman visited our hospital with the chief complaint of limited mouth opening. Clinical examinations showed severe trismus,cervical pain and dysphagia. She was diagnosed as tetanus without any apparent history of trauma after showing convulsion. The patient was performed emergency endotracheal intubation and received anti-tetanus serum and penicillin-G,and sedative against convulsion. Then tracheotomy was performed because of a long term intubation. She was recovered,and discharged from hospital. We should consider the possibility of tetanus whenever patients suffer trismus in the absence of history of trauma.
