診断に苦慮した嚥下障害の1例

Severe dysphagia in Wallenberg syndrome：A report of case

Ⅰ．はじめに

　嚥下障害は日常診療において比較的よく遭遇する症候である。嚥下障害には器質的疾患によるものと，機能的疾患によるものがある。Wallenberg症候群は一側の顔面と対側上下肢，体幹の温痛覚障害に嚥下障害，平衡障害を伴う複雑な病態を呈する。今回われわれは当初，下咽頭癌，頸部食道癌が疑われ，輪状咽頭部の通過障害のみを呈したWallenberg症候群の症例を経験したので報告する。

　This report describes a case of a patient with a right medullary infarction. A 63-year-old male had no neurological findings except severe dysphagia. Pharyngogram performed on the 1st day of admission demonstrated a bolus did not passed beyond the cricopharyngeal lesion. And enhanced CT and direct esophagoscopy demonstrated no tumor. A brain magnetic resonance imaging performed on the 17th day demoustrated a right medullary infarction,and then a patient was diagnosed with Wallenberg syndrome.
