診断に苦慮した耳下腺管異物の1例

A Case of a Foreign Body in the Parotid Duct

I．緒言

　耳下腺管異物に関する報告は稀である。しかもこれらの報告例の大半が経導管よりの侵入異物であり，体表から外傷などに起因し長期間介在した異物例はきわめて稀である。われわれは最近，約40年前の爆発事故のさいに飛散した小石片により右耳下腺管が閉塞し，以後反復性に耳下腺部の腫脹と疼痛をきたしたきわめて稀な症例を経験したので，若干の文献的考察を加えて報告する。

A 50-year-old male with recurrent parotitis resulting from a small stone in the parotid duct was reported. Calculus in the parotid duct is uncommon and foreign bodies in this duct are extremely rare. Fourty years ago, the patient was injured in his head and neck by an accident that the percussion cap of a misfired bomb bursted out when he handled it. For 35 years after the accident he had been suffering with a swelling and pain in the right parotid region recurrently. He was first consulted at our hospital on Decem-ber, 1980, and chemotherapy was given under the diagnosis of recurrent parotitis. But his symp-toms were repeated for 5 years.

On the sialographic findings and a hard mass palpable in the parotid region, exploratory oper-ation was performed under diagnosis of benign tumor. As the result, a small stone that might have invaded percutaneously on the accident was found out in the parotid duct. He became free from all the symptoms after removal of the stone.

The literature of foreign body in the parotid gland was reviewed.
