早期に診断し手術的に加療し得たPlummer病の2症例

Two Cases of Plummer's Disease―Surgical Treatment Following Early Diagnosis

I．はじめに

　Plummer病は，1913年にPlummerによりBasedow病とは異なる甲状腺機能亢進症として初めて報告された疾患である^1）。本症はTSHの支配を受けずに甲状腺ホルモンを過剰分泌する過機能性結節性甲状腺腫であり，欧米では時にみられるが^2），本邦において甲状腺機能亢進症全体に占める割合は非常に少ない^3）。

　今回，Plummer病と診断され手術的に加療し甲状腺機能が正常化した2症例を経験したので，文献的考察を加え報告する。

　Plummer's disease is characterized by the presence of autonomously functioning nodules that hypersecrete thyroid hormones,resulting in hyperthyroidism. A 60-year-old female and a 48-year-old male,with complaints of anterior neck tumor and hyperthyroid symptoms are reported. Both of the cases were promptly diagnosed by thyroid scintigraphy. Subsequent hemithyroidectomy resulted in improvement of hyperthyroidism,with only slight hypothyroidism remained in one patient. Surgery was effective in the treatment of Plummer's disease.
