頸部膿瘍を併発した抗甲状腺薬による無顆粒球症の1例

A case of antithyroid drug-induced agranulocytosis combined with deep neck infection

I．はじめに

　バセドウ病に対する治療には，薬物療法，手術，アイソトープ治療があるが，わが国では，初回治療に抗甲状腺薬による薬物治療が試みられることが多い。抗甲状腺薬による最も注意すべき副作用の1つに無顆粒球症がある。無顆粒球症を発症した場合，抗甲状腺薬の再投与は危険であり，無顆粒球症に対する治療の後，手術またはアイソトープ治療のいずれかを行うことになる。今回われわれは，頸部膿瘍を併発した無顆粒球症の治療後，無機ヨード，βブロッカー，ステロイドホルモンを用いて甲状腺機能をコントロールしたうえで，甲状腺全摘術を行った症例を経験したので若干の文献的考察を加えて報告する。

　A patient had deep neck infection due to agranulocytosis. Agranulocytosis was improved at 8 hospital day by administration of antibiotics and GCSF. Total thyroidectomy was performed after the control of thyroid functions by inorganic iodine drug,β blocker,and steroid hormone. Thyroid functions of this patient was controlled by L-T4 drug postoperatively.
