側頭骨内顔面神経鞘腫4症例の検討

Four Cases of Intratemporal Facial Neurinoma

I．はじめに

　顔面神経鞘腫は末梢性顔神麻痺の原因の約5％を占めるといわれている^1）。近年の画像検査の進歩に伴いその報告も増えており^2～5），無症候性のもので，剖検にて初めて診断されたという報告もある^6）。しかし，本疾患は進行が比較的緩徐であることから，症状が発現し診断がつくころには広範囲に進展して，侵襲の大きな手術が必要となることがある。

　今回われわれも，顔神麻痺が発現して1年以上経ってから，顔面神経鞘腫と診断され手術を行った症例を経験したので，当院で過去6年間に経験した他の3例と合わせて検討するとともに，若干の文献的考察を加えた。

　We report 4 cases of intratemporal facial neurinoma treated in our hospital.

　All cases were surgically treated with the transmastoid and transcranial approach,and nerve transplantation.

　We evaluated their preoperative and postoperative symptoms：the facial palsy,the hearing loss,and the complications. Three cases were observed for more than three years,and their facial palsy recovered to 18-point or more by the Yanagihara's method. Regardless of the kind and length of the grafted nerve,similar recovery of facial movements was obtained.
