 A 63-year-old man with a lateral medullary infarction complained of vertigo.

 He presented with spontaneous nystagmus toward the left side,but no symptoms/signs of Wallenberg's syndrome such as sensory changes,hoarseness,dysphagia and Horner sign. But delayed appearance of sensory symptoms,hiccup,and Horner sign was observed. MRI revealed an infarction of the medulla oblongata.

 As medullary infarction may present an isolated vertigo,we should be careful for diagnosis.
