両側同時性急性感音難聴として発症した小脳・脳幹多発梗塞の1症例

Cerebellar and Brain Stem Infarction with Bilateral Simultaneous Acute Sensorineural Hearing Loss―A Case Report―

I．はじめに

　急性高度感音難聴は日常診療で頻繁に遭遇し，その多くは突発性難聴として扱われる。しかし，感音難聴は蝸牛から中枢に至る聴覚路のいずれの部位が障害されても起こるため，中枢性疾患など生命を脅かす重篤な疾患の部分症である可能性を常に念頭に置き，病歴や神経学的所見に注意を払って診察を進める必要がある。

　今回われわれは，両側同時性の急性感音難聴として発症した小脳・脳幹多発梗塞の1症例を経験したので報告する。

　A 61-year-old man complained of bilateral hearing loss. The acute onset of his sensorineural hearing loss suggested sudden deafness,however,unlikely typical sudden deafness,the hearing impairment of both sides in this case was symmetrical and had simultaneous onset. The neurological and otological findings suggested central neurological disturbance. MRI showed cerebellar and brain stem infarction. Brain infarction rarely causes bilateral hearing loss,but it is important to consider central nervous disorders as the possible cause of acute hearing loss.
