角膜切開によるソフトシェル法を併用した小児先天白内障手術

Surgery for infantile congenital cataract by corneal incision and soft-shell technique

　先天白内障4例8眼に水晶体摘出手術を行った。年齢は7か月から5歳であり，術後6か月以上の経過を観察した。術式は角膜切開アプローチで，ヒーロン^®とビスコート^®を使用し，ソフトシエル法を併用して前嚢切開，水晶体吸引，後嚢吸引を行い，その部経由で前部硝子体を切除した。2眼には眼内レンズを挿入し，嚢内に固定した。術中合併症はなかった。術後合併症として2眼に後発白内障が生じたが，視力には影響しなかった。本術式は先天白内障に有用であると結論される。

We performed surgery for congenital cataract in 8 eyes of 4 patients. Their ages ranged from 7 months to 5 years. They were followed up for 6 months or longer. Surgery was performed by corneal incision, soft-shell tech-nique using Healon^® and Viscoat^®, anterior capsulotomy, lens aspiration, posterior capsulotomy, and anterior vitrec-tomy. Intraocular lens was implanted and fixated in the bag in 2 eyes. There was no intraoperative complication in the whole series. After-cataract developed in 2 eyes but did not interfere with visual acuity. The findings show that the present surgical technique is useful for congenital cataract.
