東京医科大学眼科における前部虚血性視神経症の動向

Trends of anterior ischemic optic neuropathy in our instituion

　過去6年間に当科を受診した前部虚血性視神経症15例17眼の動向を検索した。罹患側は右5眼，左12眼で，2例が両側発症であった。男性10例，女性5例で，平均年齢は，男性63.3±16.0歳，女性61.0±10.8歳であった。視力の転帰は，改善5眼，不変8眼，悪化4眼であった。初診時の視野は，水平半盲7眼（41％），中心暗点3眼（18％），傍中心暗点3眼（18％）などであり，複数回の視野検査ができた13眼での転帰は，改善2眼，不変7眼，悪化4眼であった。治療として，12眼に血管拡張薬と副腎皮質ステロイド薬の投与、3眼に血管拡張薬のみの投与，1眼に副腎皮質ステロイド薬の投与を行い，1例が無投薬であった。

We reviewed 17 eyes in 15 patients of anterior ischemic optic neuropathy seen during the foregoing 6 years. The right eye was involved in 5 cases and the left in 12 cases including 2 bilateral cases. The series comprised 10 males and 5 females. The ages averaged 63.3±16.0 years in males and 61.0±10.8 years in females. The visual acuity improved in 5 eyes, remained unchanged in 8 eyes, and deteriorated in 4 eyes. The initial visual field changes included horizontal hemianopia 7 eyes, central scotoma 3 eyes and paracentral scotoma 3 eyes among others. Perimetry could be repeated in 13 eyes. The visual field improved in 2 eyes, remained unchanged in 7 eyes, and deteriorated in 4 eyes. Systemically, 12 eyes received vasodilator and corticosteroid, 3 eyes received vasodilator alone, and 1 eye received corticosteroid alone. One eye was followed up with no medication.
