We reviewed the outcome of penetrating keratoplasty in 162 eyes of 143 patients during a ten-year period through 1994. Bullous keratoplathy was the most frequent underlying lesion at 41.4% in the series. About one-half of the cases were pseudophakic. Transparency of the cornea was obtained in 87.3% at one year and 68.8% at 5 years. Rejection of the graft occurred in 25.9%, of which graft opacity developed in 42.9%. The findings suggest that penetrating keratoplasty is to be performed with simultaneous cataract surgery, or triple procedure. We advocate use of preserved sclerocorneal graft stored in OptisoilM for keratoplasty to be performed on a systematic basis.
