Epstein-Barr virusが関与していると思われた小児視神経炎の2例

Possible association of Epstein-Barr virus in two cases of infantile optic neuritis

　近年，Epstein-Barr virus （EBV）と眼疾患の関係が注目されてきている。今回，筆者らは髄液検査よりEBVが関与していると思われた小児視神経炎の2症例を経験した。症例1は9歳男児で左眼視神経乳頭炎を認め，髄液検査を行ったところ，PCR法にてEBVのDNAが検出された。症例2は4歳男児で，無菌性髄膜炎にて入院中に右眼視神経乳頭炎を認めた。髄液検査を行ったところEBVの髄液中の抗体が陽性であり，PCR法にてEBVのDNAが検出された。また，小児の無菌性髄膜炎で視神経炎を伴わない5症例で，PCR法による髄液検査を行ったが，EBVのDNAは検出されなかった。以上のことより，視神経炎とEBVとの関連が示唆された。

We observed two boys, aged 9 and 4 years, who suffered from optic neuritis and were positive for Epstein-Barr virus (EBV) in the cerebrospinal fluid (CSF). The first case presented with acute optic papillitis. Physical findings were inconspicu-ous. DNA for EBV was detected by polymerasechain reaction (PCR) in CSF. The second case was hospitalized for aseptic meningitis with secondary optic neuritis. Serological examinations of CSF showed virus capsid antigen (VCA) IgG antibody for EBV. DNA for EBV was again detected by PCR method. No evidence for EBV was found in 5 other cases of infantile aseptic meningitis without optic neuritis. Above findings suggest that EBV may be associated in the pathogenesis of some cases of infantile optic neuritis.
