（D−8AM−9）　初回手術としてマイトマイシンC （MMC）併用線維柱帯切除術を施行した原発開放隅角緑内障50例61眼を対象に術後眼圧調整率，術後早期眼圧と長期予後の関係，眼圧長期予後に影響する因子について検討した。目標眼圧15mmHgとした場合，術後72か月の眼圧調整率は無治療で60.0％であった。術後1週間の平均眼圧15mmHg未満の群では15mmHg以上の群より有意に長期予後が良好であった。Cox比例hazard modelを用いた解析から無治療目標眼圧を15mmHg未満としたとき，眼圧長期予後に影響する因子としてMMC塗布時間が選択された。MMC併用線維柱帯切除術では術中の十分なMMC塗布と術後早期の眼圧を15mmHg未満にコントロールすることが重要と思われた。

We reviewed 61 eyes in 50 patients of primary open-angle glaucoma who underwent trabeculectomy with intraoperative mitomycin C. Control of intraocular pressure (IOP) under 15 mmHg without medication was obtained in 60.0% of eyes 72 months after surgery. Eyes with IOP below 15 mmHg one week after surgery showed significantly better long-term IOP control (p = 0.034). Analysis using Cox proportional hazard model showed application time of mitomycin C as a significant factor for good long-term outcome (p = 0.0363) . The findings show the importance of sufficient intraoperative application of mitomycin C and IOP control below 15 mmHg during the early postoperative period in achieving long-term success.
