We observed two females, aged 52 and 69 years each, who manifested rapid proptosis with impaired vision in both eyes. Both cases were diagnosed as dysthyroid optic neuropathy. Computed tomography and magnetic resonance imaging showed compression of the optic nerve at the orbital apex by hypertrophied extraocular muscles. Systemic corticosteroid and radiation proved futile. We performed bilateral orbital decompression by resecting the medial orbital wall with forced prolapse of the orbital fat tissue. Both cases responded well to surgery and the visual acuity recovered to normal in 4 to 11 weeks. Proptosis and central scotoma had also disappeared. The findings illustrate the usefulness of orbital decompression in acute dysthyroid optic neuropathy.
