涙小管に生じた化膿性肉芽腫の1例

A case of pyogenic granuloma in the lacrimal canaliculus

　眼科領域でも，霰粒腫に起因して，あるいは翼状片などに対する手術後に，化膿性肉芽腫を生じることがある。筆者らは，涙小管から突出してきた化膿性肉芽腫の1列を経験した。本症例では患側の眼瞼内眼角部に霰粒腫がみられ、この霰粒腫に起因して化膿性肉芽腫を生じたものと考えた。全摘出の比較的容易な箇所では化膿性肉芽腫の再発は少ないとされているが，管腔の狭い涙小管のような部位では腫瘤の部分的な摘出では再発が多いとされており，涙小管機能の温存をはかりながら，誘引となる霰粒腫を含めて全摘出を要するものと考えた。

A 75-year-old female presented with painful swelling in her right upper eyelid of one month' s duration. Eye discharge had increased since one week before. Hyperemic swelling was present in the inner canthal portion of the upper eyelid. A small tumor simulating chalazion was present in the superior lacrimal punctum. Staphylococcus was isolated from the pus from the lacrimal canaliculus. Systemic chemotherapy was futile and the tumor elongated to protrude outwards two months later. Surgical removal of the tumor led to the diagnosis of pyogenic granuloma. It appeared to have originated from a small chalazion in the inner canthal portion.
