ベーチェット病長期観察例の視力経過特に免疫抑制剤および白内障との関係

Longterm visual outcome in patients with Behçet disease. Implication of immunosuppressants and cataract

（展示101）　ベーチェット病50例100眼の視力経過を検討した。男子21例，女子29例であり，年齢は平均52歳であった。経過観察期間は5年から11年，平均7年2か月であった。視力0.1未満を視力不良，0.7以上を視力良好とした。視力不良例は、経過が10年以上，男子完全型，FK506またはシクロスポリンA使用例に有意に多かった。10年以上の例で視力が不良である原因としては，本病では経過とともに視力が低下すること，近年本病が軽症化していること，FK506やシクロスポリンAの出現などが考えられた。HLAB51の陽性と陰性群の間には差がなかった。白内障手術24眼のうち，視力不良が10眼あり，うち6眼がFK506またはシクロスポリンAの投与を受けていた。

We reviewed 100 eyes of 50 patients of Behçet disease regarding the visual outcome. The series comprised 21 males and 29 females. The duration of observation ranged from 5 to 11 years, average 7 years. Poor visual acuity of 20/200 or less was more frequent in males, duration of the disease for 10 years or longer, and in cases treated by systemic FK506 or cyclosporin A. Poor visual acuity in long-duration cases appeared to be due to the nature of the disease itself, to the tendency of the disease to be more benign in recent times, and to the frequent use of FK506 or cyclosporin A. There was no difference in visual outcome between cases with positive and negative HLA B51. Cataract surgery had been performed in 24 eyes, of which 10 eyes showed poor visual acuity. Six of the 10 eyes had received systemic FK506 or cyclosporin A.
