We modified the surgical procedure for cataract to reduce the surgically induced astigmatism and to correct the preexisting astigmatism. In 61 eyes with against-the-rule astigmatism of more than 1 diopter, a polymethylmethacrylate single-piece intraocular lens (IOL) was implanted through a keratocorneal tunnelling incision 6 mm in width. In 116 eyes with corneal astigmatism of 1 diopter or less on any axis, a foldable acrylic IOL was implanted through a clear-cornea incision 3.8 mm in width. At 3 months after surgery, corneal astigmatism was within 1.5 diopter in 95% in the former group and in 98% in the latter. The corrected visual acuity improved in all the eyes. Uncorrected visual acuity improved in all but one eye.
