We performed cataract surgery in 9 eyes with exfoliation syndrome. All the eyes underwent phacoemulsification and aspiration with intraocular lens implantation. Eyes with phacodonesis before surgery were excluded from the series. The eyes were characterized by poor dilatation of the pupil. There was no incidence of intraoperative complications including ruptured zonule, rupture of the posterior capsule or vitreous loss. Postoperatively, 2 eyes showed inflammatory reaction with fibrin in the anterior chamber and 4 eyes developed elevated intraocular pressure. These complications subsided spontaneously. The findings show that phacoemulsification and aspiration may be safely performed in cataractous eyes with exfoliation syndrome without phacodonesis.
