落屑症候群における超音波水晶体乳化吸引術

Phacoemulsification and aspiration in eyes with exfoliation syndrome

（26A-3）　落屑症候群を伴う白内障9眼に対し超音波水晶体乳化吸引術（PEA）および眼内レンズ挿入術を施行した。術前に水晶体振盪を認めず，他の術式を併用しなかったものを対象とした。術前散瞳径は小さかったが，チン小帯断裂，破嚢，硝子体脱出といつた術中合併症は生じなかった。術後にフィブリン反応が2眼，眼圧上昇が4眼にみられたが，重篤なものではなかった。水晶体振盪のない落屑症候群においては，白内障手術でPEAを選択することは妥当であると考えられた。

We performed cataract surgery in 9 eyes with exfoliation syndrome. All the eyes underwent phacoemulsification and aspiration with intraocular lens implantation. Eyes with phacodonesis before surgery were excluded from the series. The eyes were characterized by poor dilatation of the pupil. There was no incidence of intraoperative complications including ruptured zonule, rupture of the posterior capsule or vitreous loss. Postoperatively, 2 eyes showed inflammatory reaction with fibrin in the anterior chamber and 4 eyes developed elevated intraocular pressure. These complications subsided spontaneously. The findings show that phacoemulsification and aspiration may be safely performed in cataractous eyes with exfoliation syndrome without phacodonesis.
