眼・中枢神経系原発眼内悪性リンパ腫の臨床と病理

Clinicopathological features of intraocular malignant lymphoma of oculocerebral origin

　眼・中枢神経系原発眼内悪性リンパ腫7例13眼を経験した。ステロイド抵抗性のぶどう膜炎として発症し，強い硝子体混濁，網膜下の滲出斑が特徴的であった。眼原発は5例で，3例は硝子体手術による生検にて，1例は脈絡膜生検にて診断された。他の1例は診断が遅れ，絶対緑内障で眼球摘出を行い診断が確定した。中枢神経系原発の2例は，脳神経外科で診断が確定した。4例に放射線療法を行い，3例は消炎したが，1例は軽快しなかった。眼原発の5例中3例が初診時から3年以内に頭蓋内あるいは全身性のリンパ腫を生じ，うち2例が死亡した。中枢神経系原発のうち1例も意識障害をきたしており，眼のぶどう膜炎は放射線治療で軽快したが，全身的には予後不良と思われた。

We observed 13 eyes of 7 cases with intraocularmalignant lymphoma originating either in the eyeor the central nervous system. Characteristically,the eyes manifested chronic uveitis with massivevitreous opacity and yellow-whitish subretinalexudates. Systemic corticosteroid was futile. Diag-nosis of intraocular malignant lymphoma wasmade in 5 cases:3 by vitreous biopsy, 1 by cho-roidal biopsy and 1 by surgical enucleation. Twocases were diagnosed on the basis of brain tumor.Irradiation to the eye was effective in 3 cases. In 3cases, ocular lymphoma developed in to systemic orcentral nervous system lymphoma in 3 years, ofwhom death resulted in 2. The prognosis quoadvitam was thus very poor in lymphoma of the eyeor central nervous system.
