切迫黄斑円孔に対して硝子体手術を施行した2症例

Vitrectomy for impending macular hole

　黄斑円孔の前段階病巣は切迫黄斑円孔とよばれ，黄斑円孔に進行するのを阻止するためには硝子体手術が必要である。今回，切迫黄斑円孔の2症例に予防的硝子体手術を行った。症例1は51歳女性で，右眼に黄斑円孔を認め，左眼の切迫黄斑円孔に対して硝子体手術を行った。術中に後部硝子体剥離を作成することができ，視力も術前（0.4）から術後（0.7）に改善した。症例2は54歳女性で，右眼は正常であったが，左眼の切迫黄斑円孔に対し硝子体手術を行った。術中に後部硝子体剥離を作成することはできなかったが，micro—hooked needleを用いて黄斑部の後部硝子体膜を離断することができ，視力は術前（0.3）から術後（1.0）に改善した。

Vitrectomy was performed in 2 eyes with impend-ing macular hole. Case 1 was a 51-year-old womanwho had a macular hole in her right eye and animpending macular hole in the left eye. We perfor-med vitrectomy in her left eye and the posteriorvitreous membrane was peeled by suction with avitreous cutter probe. Case 2 was a 54-year- oldwoman who had an impending macular hole in herleft eye and the right eye was normal. We perfor-med vitrectomy in her left eye and segmentation ofthe posterior vitreous membrane with a micro-hooked needle was done. The visual acuity wasimproved after surgery in the 2 eyes.
