真菌性眼内炎の統計学的検索および治療方法について

Statistics and therapeutic guideline of fungal endo-phthalmitis

　過去78か月間に経験した内因性真菌性眼内炎38例66眼について統計的検索を行い，症例中，経静脈高栄養投与IVH装着者が87％であった。肝疾患患者における本症の増加もあり，今後の発症に注意を要すると思われた。硝子体手術時に用いる眼内灌流液にフルコナゾールを添加する場合，200μg／mlまでが安全濃度と思われた。IVH装着者の定期眼科検査を行い良好な効果を得たことから，IVH装着者のみならず，いわゆるimmuno-compromized hostの定期的眼科検査の重要性を痛感した。

We reviewed 38 cases, 66 eyes, of endogenousfungal endophthalmitis from the foregoing 78-month period. Use of intravenous hyperalimenta-tion (IVH) appeared to be the cause in 32 cases,84%. The condition was treated by vitrectomy in 23eyes, 17 cases. Addition of fluconazol to the irrigat-ing solution during vitrectomy seemed to be ofvalue up to the concentration of 200 μg/ml. Weadvocate routine ophthalmological check-up inpetients under IVH treatment.
