硝子体手術で判明した眼内悪性リンパ腫の1例

Intraocular malignant lymphoma was diagnosed in a case after vitrectomy

　原因不明の硝子体混濁で受診し，硝子体液の細胞診にて眼内悪性リンパ腫と診断された1症例を経験した。症例は73歳の女性で，糖尿病にて加療中であり，右眼視力低下にて約1年前から近医通院中であった。角膜後面に細かい沈着物と濃厚な硝子体混濁を認め，硝子体切除術を行ったところ，細胞診にて硝子体内に腫瘍細胞の浸潤を認めた。眼底には網膜深層に黄白色の滲出斑を認めた。化学療法を開始したが，眼症状の発現から約2年後に，頭蓋内転移のため死亡した。本症においては，早期診断が生命予後という点からも重要であり，中高年に発症した難治性のぶどう膜炎に対しては，本症を念頭におく必要があることを痛感した。

A 73-year-old diabetic female presented with blurred vision in the right eye of one year's duration. The affected eye showed fine keratic precipitates and dense vitreous opacity. Cytologic studies of vitreous specimen obtained by vitrectomy led to the diagnosis of malignant lymphoma. Yellowish exudateds had accumulated in deeper retina. In spite of chemotherapy, the patient died of intracranial involvement 2 years after onset of ocular signs. Malignant lymphoma is a possibility in recalcitrant uveitis in aged persons. Early diagnosis and treatment are essential for life prognosis in this condition.
