ぶどう膜炎患者における血中組織プラスミノーゲンアクチベータおよびプラスミノーゲンアクチベータインヒビタ−1の動態

Tissue plasminogen activator and plasminogen activator inhibitor-1 in the blood in uveitis patients

　組織プラスミノーゲンアクチベータ（t-PA）は各種体液，臓器に存在し，運動負荷や精神的ストレスなど様々な生理的状況下で変動し，また血栓症などの血管閉塞，ショックや手術侵襲などの病態時にも変動する。筆者らはこれまでt-PAの活性部位を阻害しない部位で結合するモノクローナル抗体SP−322を用いてt-PA活性を測定し，各種疾患における線溶動態の意義を検討してきた。

　今回は原因不明のぶどう膜炎新鮮例を対象に血中t-PA活性，t-PA抗原，PAI−1抗原を測定し，基準値および中心性漿液性網脈絡膜症患者の値と比較検討した。その結果，ぶどう膜炎患者は基準対照に比べ，t-PA活性（P＜0.05），t-PA抗原（p＜0.005），PAI−1抗原（p＜0.001）が有意に高値を示した。また中心性漿液［生網脈絡膜症患者に比べ，t-PA活性（P＜0.01）は有意に高値，PAI−1抗原（p＜0.05）は有意に低値を示した。このように血管炎を病態とするぶどう膜炎患者の　t-PAおよびPAI−1に認められる血中線溶動態はICSC患者とは異なり，凝固線溶系バランスの関連においても異なる疾患群であることが考えられた。

We quantitated the tissue plasminogen activator (t-PA), t-PA antigen and plasminogen activator inhibitor-1 (PAI-1) in 31 patients with idiopathic uveitis. The values in healthy persons and in patients with idiopathic central serous chorio-retinopathy (ICSC) served as control. Whencompared with healthy persons, there were signifi-cant increases in t-PA activity (p＜0.05), t-PA antigen (p＜0.005) and PAI-1 antigen (p＜0.001). When compared with ICSC, there was a significant increase in t-PA (p＜0.01) and decrease in PAI-1 (p＜0.05). The findings seemed to show that idiopathic uveitis and ICSC are different disease entities regarding fibrinolytic and coagulative bal-ance in the circulating blood.
