　45歳男性が左眼の霧視を主訴に来診。左眼に著しい硝子体混濁を認めた。全身的ステロイド療法は無効であった。硝子体吸引と硝子体手術を施行し，細胞診および免疫組織学的検索の結果よりClassV, B細胞型の悪性リンパ腫と診断された。眼底には明らかな異常所見はなかった。全身検索により眼以外には異常はみられず，眼原発悪性リンパ腫と診断した。化学療法と髄腔内投与後に予防的全脳照射を含めた放射線療法を施行し，良好な経過をたどっている。細胞診による早期診断ならびに化学療法と放射線療法の併用が有効と考えられた。

A 45-year-old male presented with vitreous opac-ity with reduced vision in his left eye. Systemic corticosteroid was futile. B cell type malignant lymphoma was diagnosed by immunocytochemical analysis of material obtained during vitrectomy. The fundus appeared normal after surgery. No extraocular lesions could be detected. He was treat-ed by systemic and intrathecal chemotherapy fol-lowed by radiotherapy to the brain and the eye. He has been in the state of complete remission during one year.
