超音波乳化吸引術および眼内レンズ移植術後の角膜乱視の推移を,切開創の長さ7mm,6mmおよび4mmに分けて検討した。7mm群,6mm群,4mm群にはそれぞれ端々縫合を3糸,2糸,1糸おいた。7,6mm群にはPMMAレンズを,4mm群にはシリコンレンズを移植した。角膜乱視はオートケラトメーターを用いて,術前,術後1,2週,1,2,3,6か月に測定し,倍角座標法により解析した。その結果,術後乱視変化量,倍角座標水平成分による直乱視化の程度とも 4mm切開群が最小で,ばらつきも小さく,また術後2週以後乱視が安定していた。4mm切開による眼内レンズ移植術は,術後早期からの乱視軽減に有効な一法と考えられた。
We reviewed the outcome of cataract surgery in131 eyes during the foregoing 6-month period. We paid particular attention to the postoperative astig- matism dependent on different sutures and length- of corneal incision. The present series comprised three groups A, B and C. The corneal incision was 7, 6 and 4 mm for group A, B and C respectively. We placed 3, 2 and 1 interrupted sutures with 10-0nylon for each group. Eyes in group A and B were implanted with PMMA posterior chamber lens. Eyes in group C were implanted with silicone poste-rior chamber lens. We performed keratometry up to 6 months postoperatively. The magnitude of astigmatism was less in group C than in the other 2 groups throughout the postoperative period. The findings show that 4 mm corneal incision with sili-cone intraocular lens is effective in minimizing postoperative astigmatism in cataract surgery.
