20年以上罹病の糖尿病患者の白内障術後結果の検討

Outcome of cataract surgery in patients with diabetes mellitus of more than twenty years' duration

　20年以上糖尿病歴のある患者で白内障手術を受けた18例26眼について検討した。その結果20年以上糖尿病歴のある患者の白内障のほうが老人性白内障よりも早く手術する傾向があることがわかった。白内障に対し，嚢内摘出術，嚢外摘出術，IOL挿入術併用嚢外摘出術，超音波乳化吸引術などを行った。術後視力をみると，最終診察時まではほとんど変化がなかったのに対し，術後網膜症は術後12か月までは改善傾向を示すが，その後は悪化傾向を示した。視力と網膜症の長期予後は，嚢外摘出術またはIOL挿入術併用嚢外摘出術のほうが嚢内摘出術よりも良好であった。レーザー光凝固は手術前および術後6か月以内に行うのが望ましいと考えられた。

We evaluated the clinical features of cataract in patients who had diabetes mellitus for 20 years or more. Cataract was found in 89 persons, 50 males and 39 females. Cataract surgery was performed on 26 eyes of 18 patients: 14 eyes of 10 males and 12 eyes of 8 females. The average at surgery was 60 years for males and 61.6 for females. The duration of diabetes averaged 18.2 years: 18.3 for males and 17.8 for females. The cataract appeared as mixed senile and diabetic one. Posterior lens im-plantation was performed in 5 of the 26 eyes. Postoperative visual acuity improved in 14 eyes, remained unchanged in 4 and decreased in 6. The postoperative state of diabetic retinopathy im-proved in 3 eyes, remained unchanged in 14 and deteriorated in 7.
