細隙灯顕微鏡写真撮影によって発生した重篤な網膜光障害の2例

Two cases of severe photic retinopathy following slitlamp photography

　偽水晶体眼に対して行った細隙灯顕微鏡写真撮影にさいして発生した，重篤な網膜光障害の2症例を報告した。両症例とも白内障手術後，良好な視力改善が得られていたが，細隙灯顕微鏡による徹照写真撮影直後より急激な視力障害を訴えた。眼底後極部には検眼鏡的に1乳頭径大の白色円形の変色斑が多数観察され，あたかも網膜光凝固斑のごとき所見を呈していた。症例1の右眼では変色斑が黄斑部領域に集中したため，高度の視力低下をきたした。症例1の左眼および症例2の右眼では変色斑が傍黄斑部領域にとどまり，視力への影響はごく軽微であった。事故が発生した背景には、撮影条件として，照明光を完全同軸で眼内に照射したこと，照射輝度が最大であったこと，しぼりが全開であったことなど，使用方法が必ずしも適正でなかったことが大きいと考えられた。使用したスリットランプの照明系についても検討の余地があると考えられた。

Retinal photic injury developed in 2 eyes immedi-ately after slitlamp color photography of the ante-rior ocular segment. Both eyes had undergone cata-ract surgery by phacoemulsification and posterior lens implantation. The patients complained of severe visual disturbance after photography. Oph-thalmoscopy showed numerous white spots resem-bling those after photocoagulation in the fundus. These cases pose two problems: adequate tech-nique of slitlamp photography with flash light and determination of flash power to avoid possible photic injury in the retina.
