角膜真菌症の治療経過中にtrophic ulcerと考えられる状態を呈した1例

A case of corneal mycosis developed presumed trophic ulcer

　68歳の女性で1年前からのソフトコンタクトレンズ連続装用中，右眼に角膜真菌症が発症した。イトラコナゾール内服による治療を発症4週後に開始した。真菌症は治癒したと判定されたが，角膜上皮欠損は遷延し，trophic ulcerと考えられる病像を呈し，角膜穿孔を合併した。治療用ソフトコンタクトレンズの装用では改善されず，細胞接着性糖蛋白質であるビトロネクチン点眼により治癒せしめることができた。

A 68-year-old female presented with corneal mycosis in the right eye after continuous wear or soft contact lens. Search for eumycetes was nega-tive on day 10 of the disease, presumably due to prior oral itraconazol treatment. Corneal epithelial defect persisted in spite of various treatments, suggesting the nature of the lesion to be trophic ulcer. Instillation of vitronectin eye solution, a kind of cellular adhesive glycoproteins, was initiated on 49 day of disease. The corneal lesion started to gradually heal from day 14 of vitronectin treat-ment. The whole corneal lesion was recovered by corneal epithelium on day 50 of treatment.
