フルコナゾールの局所投与により治癒した角膜真菌症の1例

A case of keratomycosis cured by topical fluconazole

　外傷に起因する角膜真菌症が57歳の男性に発症した。当初ピマリシン点眼，5-FCの内服を試みたが効果がなく，5-FCによる薬疹が出現した。ついでイトラコナゾール内服で治療を行ったが，やはり薬疹が出現したため，内服治療は中止した。患者はフルコナゾールの結膜下注射，点眼および病巣部の掻爬（debridement）による治療に反応し，角膜中央に淡い混濁を残して治癒した。視力は眼前指数弁より0.3まで回復した。フルコナゾール局所投与および病巣掻爬による副作用は全く認められなかった。

A 57-year-old male developed keratomycosis in his right eye after accidentally scratching the cor-nea. He was initially treated with oral flucytosine and topical pimaricin. Oral flucytosine had to be discontinued due to skin eruptions. Oral itracon-azole was also dropped for the same reason. We started treatment with surgical debridement and topical fluconazole 4 weeks after onset of the disease. Prompt improvement set in. Signs of cor-neal infection disappeared 6 weeks later with in-provement of visual acuity from conting fingers to 0.3. No adverse reactions were noted during the treatment.
