コンタクトレンズ非装用者にみられたアカントアメーバ角膜炎の1症例

Acanthamoeba keratitis in a contact lens non -wearer

　コンタクトレンズ非装用者に発症したアカントアメーバ角膜炎の1例を報告した。症例は39歳の男性で，角膜ヘルペスとして治療を受けていたが，症状が改善せず，角膜の生検によってアカントアメーバが証明され，アカントアメーバ角膜炎と診断された。培養されたアカントアメーバはA．culbertsoniと同定され，本邦では初例であった。治療はフルコナゾールの内服，ミコナゾールの点眼および病巣掻爬の三者併用を用いて良好な結果を得た。コンタクトレンズ装用者以外に発症したアカントアメーバ角膜炎は本邦では他に報告がなく，角膜の外傷の先行や土壌からの感染などが推察された。

A 39-year-old man developed corneal infection simulating disciform keratitis. He underwent treat-ment with antiviral agents and corticosteroid under the tentative diagnosis of herpetic keratitis. He was referred to us due to exacerbation after 4 months of treatment. Cytological studies of biopsied corneashowed Acanthamoeba with ink-potassium hydro-xide staining. Cultured scraping materials yielded Acanthamoeba culbertsoni, which has not as yet been identified from Acanthamoeba keratitis in Japan. He was then treated with oral fluconazole, topical miconazole and surgical debridement of corneal lesion. These treatments were effective and were well-tolerated. This case is, to our best know-ledge, the first reported one of Acanthamoeba keratitis in contact lens non-wearer in Japan. This patient presumably suffered from mild corneal injury and was exposed to contaminated soil prior to manifest corneal infection.
